The initial consultation with your medical oncologist is a comprehensive evaluation of your history and medical records. This consultation may last up to an hour or longer.

Prior to this visit, a new patient packet will be mailed to you for completion. This new patient packet will include demographic information, insurance information, HIPAA information, and a medical history questionnaire.

During the visit, you will have the opportunity to have your questions answered. We strongly encourage loved ones, caregivers, and family members to be in attendance with the patient at this meeting.

Please plan to arrive a minimum of 30 minutes prior to your scheduled appointment time.

You will receive automated appointment reminders by email, text, or voicemail within 5 days of your scheduled office visit with us.  Please CONFIRM your appointment upon receiving these messages OR select the option to cancel or reschedule.  Please do not reply STOP to block these messages as it will discontinue all future appointment reminders.

• Completed New Patient Packet
• Medication List
• Up-to-date Insurance Card
• License or photo ID, prescription card
• Any applicable co-pay
• List of questions or concerns to discuss with your doctor
