We use the outcomes data provided to CCAY from this program to evaluate our processes towards continuous quality improvement initiatives of care.
The integration of these enhanced services are provided to our OCM participants at NO ADDITIONAL COST!
All OCM Beneficiaries receive a survey from the Centers for Medicare and Medicaid Services (CMS). Your feedback is important and imperative in our decision making towards improved healthcare practices. Your voice is welcomed and is vital to making this program a success.
